We would like to take this opportunity to welcome you to our office and to assure you that EVU Center is committed to providing you with the best possible care.
Please read the following information regarding our financial policy. We will gladly discuss your proposed treatment and assist you with any questions relating to your insurance.
Please bring your insurance cards with you to each visit, as there are often updates in addresses or changes in your plan that we must know in order to file your claims correctly. If you do not have insurance, payment for services is expected at the time of service, unless payment arrangements have been made in advance. To assist you, we accept cash, check, MasterCard, Visa, Discover Card, and American Express. If you don’t have your insurance cards at the time of your visit, you may be asked to reschedule your appointment. Please come prepared with all the current information.
If you have insurance coverage, as a courtesy we will file ALL insurance claims when an assignment of benefits is given. If we do not participate with your insurance plan, you are responsible for following up with your insurance company regarding the payment of your claim. Your coverage is a contract between you and your insurance company. If we do participate with your primary insurance, you will receive a statement for any balance due AFTER payment is received from your insurance company. Please remember that not all services are a covered benefit in all contracts. Some insurance companies arbitrarily select specific services they will NOT cover.
All outstanding balances are due within 30 days, unless prior arrangements have been made with the billing department. All balances that reach 90 days past due with no activity will be sent to a collection agency. Should your account be sent to a collection agency, you will be financially responsible for all collection fees and legal fees that our office incurs through the process utilized to collect the outstanding delinquent balance.
Payment in full of any past due balance is expected prior to being seen in our office in the future. In addition, payment in full will be expected at the time of service for any future services.
We realize temporary financial problems may affect timely payment of your account. If such problems do arise, please contact us promptly for assistance in the management of your account. We do use outside agencies as a means of collection should we deem it necessary. If you have any questions about the above information, or any uncertainty regarding insurance coverage, do not hesitate to ask us. We are here to help you.
Insurances accepted
EVU Center is a participating Medicare provider. We also accept most private insurance plans. Please always verify if we are considered in-network or out-of-network for your individual plan.
Cash pay
Many of the procedures and medications that are prescribed in our Health and Wellness practice are NOT covered by insurance. We try to deliver our products quickly, safely, and in a cost-efficient manner, which tends to reduce the out-of-pocket expense for our patients. Please ask about the opportunity for bulk pricing and/or cash pay discounts.
